My friend from Mumbai used the Visa on arrival facility (Single Entry for Social Visit) at the Kuala Lumpur International Airport (KLIA) just two weeks ago. The fee is RM100 (approx. US$29).
When my friend re-entered Southern Malaysia after touring Singapore, he had to go to the Tanjung Kupang Immigration Office, located at the Second Causeway for visa on arrival.
Please take note that there is NO facilities for visa on arrival at the Johor Bahru Immigration if you enter Southern Malaysia via Singapore Woodland Causeway. Please see the link http://www.imi.gov.my/eng/perkhidmatan/im_VisaOnArrival.asp

Incase you are flying from Mumbai / Bangalore to KUL then the you can avail visa on arrival. But if you are travelling from Chennai you need to get it in advance. Its always advisable to get the visa stamping in advance.
